Does anyone know where I might find the proper forms that I will need? (free?--as I am a senior citizen, widow, on a fixed income and need every penny I have.) I have a wonderful dog, "Honey". She was supposed to be a Beagle, but I now believe she is Beagle/Pointer, as when she stands, she is about a foot taller than my kitchen cabinets. She had been neglected and abused and brought to a vet to be euthanized at the young age of 3 months. Of course, I have spoiled her rotten. (smile.) She loves everyone and every dog and I thought it would be nice for to have other dogs as companions. I have a fenced-in dog exercise area so it would be great for both she and other dogs.
